Evelyn Wells provides non-medical home care services for aging adults in the Indianapolis, Indiana area. Our services include, but are not limited to, companion care, daily living support, personalized care planning, and ongoing family communication.

Our care system — built around the principles of Stabilize, Observe, Adjust, and Sustain — is designed to provide proactive, consistent support that keeps your loved one safely at home and your family informed at every step.

We do not provide medical care, skilled nursing, or any services that require a licensed medical professional. For medical needs, we encourage families to consult a qualified healthcare provider.

Section 02

Free Consultations

Evelyn Wells offers free 30-minute consultations to prospective clients. These calls are informational in nature and do not constitute a service agreement or guarantee of placement. No payment is required to schedule or attend a consultation.

By scheduling a consultation, you consent to be contacted by Evelyn Wells via phone, email, and/or SMS regarding your inquiry and related care services.

Section 03

Client Responsibilities

Clients and their families agree to provide accurate and complete information about the care recipient's needs, health conditions (to the extent relevant to non-medical care), living situation, and preferences.

Clients are responsible for maintaining a safe home environment for Evelyn Wells caregivers. Any changes in the care recipient's condition that may affect the care plan should be communicated to us promptly.

Payment Terms

Service fees will be discussed and agreed upon prior to the commencement of care. All pricing is provided in writing before any services begin. Evelyn Wells does not publish pricing on its website, as care plans are personalized to each client's needs.

Payment terms, billing cycles, and accepted payment methods will be outlined in your individual service agreement. Non-payment may result in suspension or termination of services.

Evelyn Wells accepts private pay and long-term care insurance. Please inquire during your consultation about specific coverage and eligibility.

No Guarantee of Specific Outcomes

While Evelyn Wells is committed to delivering high-quality, proactive care, we cannot guarantee specific health outcomes, recovery timelines, or the prevention of medical events. Results may vary based on the individual needs, health status, and circumstances of each care recipient.

Our goal is to provide consistent, attentive, non-medical support that promotes wellbeing, safety, and independence at home.

Intellectual Property

All content on the Evelyn Wells website and in our materials — including but not limited to the Aging Navigation Framework™, care system documentation, brand assets, written content, and design — is the intellectual property of Evelyn Wells. Clients and visitors may not reproduce, distribute, or use this content for commercial purposes without prior written consent.

Privacy & Data Collection

Evelyn Wells collects personal information — including name, phone number, email address, and care-related details — for the purpose of providing and improving our services. We do not sell, rent, or share your personal information with third parties for marketing purposes.

Information submitted through our scheduling forms, lead forms, and consultation requests is used solely to facilitate communication and deliver care services. Please review our Privacy Policy for full details on how we handle your data.

SMS Communications & Text Messaging Terms

By submitting your phone number through any Evelyn Wells form, landing page, or scheduling tool, you agree to receive SMS text messages from Evelyn Wells. These messages may include:

• Appointment confirmations and reminders

• Follow-up messages regarding your inquiry

• Care-related updates and information

• Responses to questions you have submitted

Program Description: Evelyn Wells sends SMS messages related to home care consultations, appointment scheduling, care updates, and follow-up communications for families seeking non-medical home care services in Indianapolis, Indiana.

Message Frequency: Message frequency varies based on your inquiry and care journey. You may receive up to 5 messages per week during active follow-up periods.

Message & Data Rates: Message and data rates may apply. Contact your wireless provider for details about your plan.

To Opt Out: You may cancel SMS communications at any time. Simply reply STOP to any message you receive from us. Upon sending "STOP," we will confirm your unsubscribe status via SMS. You will no longer receive SMS messages from Evelyn Wells after confirmation. To re-subscribe, simply text START to the same number or re-submit your information through our website.

Carrier Liability: Carriers are not liable for delayed or undelivered messages.

Section 09

Limitation of Liability

Evelyn Wells is not liable for indirect, incidental, or consequential damages arising from the use of our website, services, or communications. Our total liability to any client shall not exceed the amount paid for services in the three months preceding any claim.

Evelyn Wells is not a medical provider and assumes no liability for medical decisions, health outcomes, or emergencies. In the event of a medical emergency, clients should contact 911 or a qualified medical professional immediately.

Section 10

Termination of Services

Either party may terminate a service agreement with 14 days' written notice. Evelyn Wells reserves the right to terminate services immediately in cases where the safety of our caregivers cannot be ensured, or where a client has materially breached these terms.

Upon termination, any prepaid fees for undelivered services will be refunded on a prorated basis.

Section 11

Governing Law

These Terms and Conditions are governed by the laws of the State of Indiana, United States. Any disputes arising under these terms shall be resolved exclusively in the courts of Marion County, Indiana.

Section 12

Amendments

Evelyn Wells reserves the right to update or amend these Terms and Conditions at any time. Continued use of our website or services following the posting of updated terms constitutes your acceptance of those changes. We encourage you to review this page periodically.
